So building on the respawn timers - the mob spawns are VERY close together and in many cases spawn right on top of you, making missile PCs very difficult and potentially dangerous for casters as well. Also, they are densely packed on many areas making it difficult to get in and out anywhere in a timely manner, even with lower level mobs with no chance of killing you. After a bit, I just decided to use Improved/Normal Invisibility and bypass them altogether after killing a few groups. The mob respawn timers are VERY short, most seem to be in the 2-5 minutes range, again which makes gaining xp nice, but with the top end xp at 2400 per kill, it's already super fast anyway, but makes travelling across maps without invisibility very tedious at levels 35-40.
For example the plains of sorrow map for going from town to Zero Midnight, you probably had to fight through 20 groups of spawns, each taking 30-60 seconds to kill. 10-20 minutes on 1 map of monsters that have zero chance of killing a properly equipped level 40 was meh to me, so I just invisibled past them after the first time.

my first PC was a classic cleric 26 bard 4 RDD10 str-based melee buffer (Blood and Faith if you want to look it up). At no point did my PC ever come close to dying. After the first couple levels when I got enough cash to buy some real gear at the shop in Jova, I didn't have any trouble (e.g. take damage more then 25% of my HP) until a few of the highend spider fiends on the way to the drow areas. Once getting gear in the drow merchant, the lowest my HP total went was in once against a greater balor (maybe 25% from full) and against the final human Noctu form (maybe 15% from full).
Now that build has a ton of buffs that help negate a lot of attacks (e.g. death ward prevents quivering palm, nep protects vs harm and extra negative dmg, the elemental buffs help against initial spell blatss, rdd makes fire useless, etc), but I went hunting for a challenge and couldn't find one. Now it's possible I didn't find any of the epic areas, but I'd wandered around across the dragon coast until it wrapped around back to plains of sorrow. I didn't take the boat into the underworld since it said you couldn't come back, but maybe that's it...

Now all that aside, I've quite enjoyed playing and leveling up my 2 PCs I created there (the one above and a Bard 24 Ftr6 RDD10), so please take the above as constructuve criticism. It would probably be more challenging with a more average build, so maybe that's a route I'll take.
