Re-skinning Tilesets?

I know it's possible, (I think) but I have no idea how to go about doing it. Does anyone know how I could either re-skin or combine tilesets?
Being able to duplicate them would probably be a good idea as well, that way I don't end up missing some...

One tool to help with the duplication / renaming bit is the
TileSet Duplicator
by Labtek, found on the vault.
