24 by 24 is about as big an area as you can make without the inhabitants causing too much lag. There are ways to go larger still (by segmenting off the map into different populated areas) - I am working on a 30 by 30 city right now using this technique (screenshot
here).
You don't need to make a city vast to make it interesting or realistic. Medieval cities were usually quite compact - get on Google Maps and look at satellite views of the old city of Dubrovnik, Croatia or Carcassonne, France for a good examples.
