According to the Wiki, the DC for the Discipline skill check is the opponent's attack roll.
Is that the modified or unmodified attack roll?

Modified by what? All attack rolls use the
Attack bonus
which includes all the normal components. I don't think I've even ever encountered an "unmodified" attack roll. Unless you are referring to something else entirely?

I was reading the roll as the result of the d20. Unmodified attack rolls are used to determine always hitting on 20 and always missing on 1.
